Headache and flushing, which affect a meaningful percentage of patients at standard doses, become more prevalent and more intense at 100 mg. More seriously, the risk of symptomatic hypotension — particularly when alcohol or interacting medications are present — increases in a dose-dependent manner.

Patients who experience an unsatisfying response to their initial dose are often better served by revisiting the conditions under which they took the medication — timing relative to meals, level of sexual stimulation, anxiety, and alcohol consumption — rather than immediately escalating the dose.

Why Your Doctor's Conservative Prescription May Be Precisely Right

Physicians who prescribe sildenafil at the lower end of the therapeutic range are not being cautious to the point of ineffectiveness. They are frequently making individualized judgments based on your complete clinical picture: your other medications, your cardiovascular history, your age, and your reported health status. A 25 mg prescription for an older patient on an antihypertensive is not an undertreatment — it is a calibrated starting point designed to identify your effective threshold without unnecessary exposure to side effects.

The appropriate clinical approach is one of gradual titration: begin at a dose likely to produce some benefit while minimizing risk, assess the response under consistent and appropriate conditions, and adjust methodically. This process requires patience, but it is far more likely to arrive at your genuine sweet spot than an aggressive initial escalation.

Communicating Effectively With Your Prescriber

If you believe your current dose is not meeting your needs, the most productive step is an honest, detailed conversation with your physician rather than self-adjustment. Describe not just whether the medication worked, but the specific conditions under which you took it — the timing, your dietary choices that day, your stress level, and any alcohol consumption. This information allows your physician to determine whether a dose adjustment is genuinely warranted or whether modifiable factors are the more likely explanation for a suboptimal response.

Your physician may also consider whether a switch to a different PDE5 inhibitor, such as tadalafil with its longer duration of action, might better suit your lifestyle and physiology — a consideration that is entirely separate from dose magnitude.
